- Must be at least 6 months from 2nd dose in order to receive 3rd dose. (2nd dose date of April 28)
- Bring completed/signed consent form
- Form can be completed online and print off, or completed on site.
- Bring copy of driver’s license
- Bring copy of insurance card
- Bring CDC vaccination record card that shows previous 2 shots
