There is still time to register online for the vaccine booster and flu shots through Orland Township.

Friday, October 29th, from 9:00 AM to 1:00 PM At Orland Township, 14807 South Ravinia Ave. Orland Park, IL 60462

BOOSTER REQUIREMENTS

    • Must be at least 6 months from 2nd dose in order to receive 3rd dose. (2nd dose date of April 28)
    • Bring completed/signed consent form
    • Form can be completed online and print off, or completed on site.
    • Bring copy of driver’s license
    • Bring copy of insurance card
    • Bring CDC vaccination record card that shows previous 2 shots
